HGV / LGV Driver Medicals

 

Takes about 15 – 20 minutes for examination

£50 fees (cash only)

Payment receipt will be sent to email on request

 

HGV and LGV drivers should undergo a medical examination before applying for a HGV / LGV drivers licence or at periodic renewals. You will be examined by our doctors based on DVLA D4 standards and provide you with a completed D4 medical form to send it to DVLA.

Things to bring for your HGV/ LGV driver medical appointment:

  • D4 form sent by DVLA
  • Photo ID (Driving Licence or Passport)
  • Eye glasses or eye lenses (if you wear them for driving)
  • Medication list (if you are taking any)
  • Medical notes / letter from GP (if you have any medical conditions)

PCV Driver Medicals

 

Takes about 15 – 20 minutes for examination

£50 fees (cash only)

Payment receipt will be sent to email on request

 

PCV drivers should undergo a medical examination before applying for a PCV drivers licence or at periodic renewals. You will be examined by our doctors based on DVLA D4 standards and provide you with a completed D4 medical form to send it to DVLA.

Things to bring for your PCV Driver Medical appointment:

  • D4 form sent by DVLA
  • Photo ID (Driving Licence or Passport)
  • Eye glasses or eye lenses (if you wear them for driving)
  • Medication list (if you are taking any)
  • Medical notes / letter from GP (if you have any medical conditions)

Taxi Driver Medicals

 

Takes about 15 – 20 minutes for examination

£50 fees without medical records (cash only)

£60 fees with medical records (cash only)

Payment receipt will be sent to email on request 

 

Taxi drivers should undergo a medical examination before applying for a Taxi drivers licence or at periodic renewals. You will be examined by our doctors based on DVLA D4 standards and provide you with a completed medical form for your application.

Things to bring for your Taxi Driver Medical appointment:

  • Medical form sent by your council
  • Photo ID (Driving Licence or Passport)
  • Eye glasses or eye lenses (if you wear them for driving)
  • Medication list (if you are taking any)
  • Complete medical records from your GP (if your form says that “the doctor who is filling the form should have access to your medical records”). An additional fee of £10 will be collected for appointments with medical records

NOTE: Please check with your council if they accept medical examinations from other than your registered GP before booking your appointment.

 

Crane/Fork Lift Operator Medicals

 

Takes about 15 – 20 minutes for examination

£100 fees with certificate (cash only)

Certificate will be sent to email on sameday if you pass the medicals

Certificate valid for 1 (one) year

Payment receipt will be sent to email on request

Crane/Fork Lift Operators should undergo a medical examination before applying for a licence or at periodic renewals. You will be examined by our doctors based on DVLA D4 standards and provide you with a medical certificate for your application. Our certificate is valid for one year from the date of examination.

Things to bring for your Crane/Fork Lift Medical appointment:

  • Photo ID (Driving Licence or Passport)
  • Eye glasses or eye lens (if you wear them for driving)
  • Medication list (if you are taking any)
  • Medical notes / letter from GP (if you have any medical conditions)

Racing Driver Medicals

 

Takes about 15 – 20 minutes for medical only examination and over 60 minutes for ECG appointments.

£50 fees for medical only examination (cash only)

Racing drivers should undergo a medical examination before at racing events or at periodic renewals. You will be examined by our doctors based on DVLA D4 standards and provide you with a completed medical form / certificate.

Things to bring for your Racing Driver Medical appointment:

  • Photo ID (Driving Licence or Passport)
  • Eye glasses or eye lens (if you wear them for driving)
  • Medication list (if you are taking any)
  • Complete medical records from your GP (if your form says that “the doctor who is filling the form should have access to your medical records”)

Safety Critical Medicals

 

Takes about 15 – 20 minutes for examination

£100 fees with certificate (cash only)

Certificate will be sent to email on sameday if you pass the medicals

Certificate valid for 1 (one) year

Payment receipt will be sent to email on request

Industrial and Manual workers should undergo a medical examination before starting to work at industrial sites or where ever the rule applies. You will be examined by our doctors based on DVLA D4 standards and provide you with a certificate for your employers. Our certificate is valid for one year from the date of examination.

Things to bring for your Safety critical Medical appointment:

  • Photo ID (Driving Licence or Passport)
  • Eye glasses or eye lens (if you wear them for driving)
  • Medication list (if you are taking any)
  • Complete medical records from your GP (if your form says that “the doctor who is filling the form should have access to your medical records”)
